This form is used to request Makerspace services for item creation.

Before submitting this form, please note that the information you provide will be used to obtain any required approvals and verify available funding. Please answer all questions thoroughly and avoid placeholder or incomplete responses.

Projects submitted on behalf of university departments will need to be approved prior to order completion and approval will be sent to university department/offices and program chairs/directors.

Additionally, projects submitted on behalf of student organizations require budget approval for all student clubs. This request will be sent to Pat Bristor for approval before work can begin.
